Make sure you include the
following supporting documentation
with your faxed application:

Blank Voided Check
(must be pre-printed, no starter
checks).

Copy of Driver's License
(lighten and enlarge before faxing).

Copy of Business License
or Sales Tax Permit.

If you are currently accepting
credit cards, please include 3
months processing statements
for auto approval.

Home-based businesses will
also need to fax a sample ad
showing your method of
advertising.
